The accelerating digital transformation of healthcare requires managerial leaders who can effectively integrate technology into strategic and operational decisionmaking. This study investigates the role of digital competencies of healthcare managers in fostering competitive advantage within the healthcare sector of the Republic of Kazakhstan. Drawing on dynamic capabilities theory, human capital theory, and resource-based view, the research positions managerial digital competencies as a key enabler of perceived competitive advantage. A quantitative survey design was employed, collecting data from 140 healthcare managers across hospitals and clinics in Kazakhstan using a four-block, 34-item Likertscale instrument. Block 1 assessed general digital competencies, Block 2 covered the application of digital tools in work, Block 3 captured perceived impact on managerial effectiveness, and Block 4 examined organisational opportunities for digital skill application. Blocks 1–2 operationalised the independent variable (DC: digital competency composite) and Blocks 3–4 the dependent variable (CA: perceived competitive advantage composite). Statistical analyses included descriptive statistics, reliability analysis (Cronbach's alpha), Spearman's rank correlation, and simple linear regression. All scales demonstrated excellent internal consistency (α = 0.879–0.945). The DC–CA correlation was moderate (ρ = 0.490, p < .001), and the regression model was statistically significant (β = 0.551, R² = .307, p < .001), explaining 30.7% of the variance in perceived competitive advantage. The findings support H1: managerial digital competencies are positively and significantly associated with perceived competitive advantage in Kazakhstan's healthcare organisations. The study highlights digital tool application (Block 2, M = 3.17) and organisational enabling conditions (Block 4, M = 3.21) as priority areas for investment. Limitations include convenience sampling, cross-sectional design, common method bias risk, and reliance on perceived rather than objective competitive advantage measures. vii This research fills a contextual gap by delivering empirical insights from a Central Asian healthcare system undergoing digital transformation and provides a validated measurement instrument for future research.
